Registration/Lottery Guidelines
Registration forms are accepted at all times and may be picked up at the school office between the hours of 8:30 A.M. and 4:30 P.M Once a registration form is completed and signed, it is placed with all registration forms for that grade level. Once an opening is available there is a drawing from that grade level.
Completing the Application

An application packet must be completely filled out for each student. |

When submitting the application we must have a copy of the child’s birth certificate, social security card, a copy of their shot records, and proof of address. All medical Forms requiring a doctors signature must be complete. |

Upon submission of the completed application a placement exam will be given and/or scheduled. |
|
|
Applications along with the placement exam results will be entered for the lottery process. |
|
|
All new students entering CCPA for the first time, and all new or return Seventh grade students are required to have a Physical Form completed by their doctor prior to admission. |
Bus Transportation
A bus vendor provided by the local public school district will transport CCPA students to and from school and/or a day care provider as long as it is within the Cincinnati Public School District.
